Client Intake Team
At Support Foundation, our Client Intake Team plays a vital role in ensuring that individuals seeking support receive the right services tailored to their needs. As the first point of contact, the team provides a welcoming, compassionate, and professional approach to help clients navigate our programs and access essential support. Our intake specialists conduct initial assessments to understand each client’s unique situation, whether they require NDIS support, homelessness assistance, mental health services, or crisis intervention. This process includes gathering essential information, identifying immediate needs, and determining eligibility for various programs.
Support Coordination Team
Once a client’s needs are assessed, our intake team works to connect them with the appropriate resources, our Support Coordinators. This may involve social workers, welfare worker, team leader, or housing manager. The team ensures a smooth transition into support services, helping clients feel empowered and supported from the very beginning. We guarantee the reach out time from our coordination team to the client is within 2 hours and service implementation is done within 24 hrs of referral. If adjustments are required, we assist clients in requesting plan modifications or accessing additional funding. This can be done at any time by calling our office or reaching out to your assigned coordinator. Alternatively, monthly follow-up call or email will be done to all the participants to seek any feedback.
Service Implementation Team
At Support Foundation, our Service Implementation Team is responsible for ensuring that participants receive effective, personalized, and well-coordinated support services in line with their NDIS plans. Our team works closely with support coordinators, clients, families, and other service providers to implement tailored care plans that promote independence, inclusion, and quality of life. Once the referral is accepted by the service implementation team, the services are implemented within 24 hrs subject to availability. Our team also conducts regular check-ins, progress of the service delivered, assessments, and plan reviews (if needed) to ensure that support services remain effective and relevant.
