NDIS Services
Support Foundation is a registered NDIS provider (4050064716) with a specialist focus on forensic disability and justice transition support. We provide person-centred disability support for participants requiring Core Supports, Supported Independent Living, justice and forensic transition support, suitable accommodation pathways and coordinated support following hospital discharge. Our services cover Sydney, Melbourne, Canberra and Adelaide.
Specialist focus: Forensic Disability & Justice Transition Support
Disability-related NDIS supports for eligible participants who are involved with, or transitioning from, the justice system. We work alongside justice agencies, health services and support coordinators to support safe transitions, daily living, community participation, accommodation pathways and greater independence.

Allied Health & Therapeutic Supports

Support coordination: typical hours
The following are the typical minimum hours our Support Coordinators take to complete these services, excluding travel:
| SDA house assessment: 20 – 25 hrs | SIL / SDA combined: 30 – 35 hrs |
| SIL house assessment: 15 – 20 hrs | ILO house assessment: 15 – 20 hrs |
| Plan review assessment: 10 – 15 hrs | Support Coordinator (home visit): 2 hrs |
| Support Coordinator (case conference): 1 hr | Email or phone call: billed in 15-minute blocks |
Referrals and scope
Support Foundation assesses every referral individually. We only accept supports that fall within our NDIS registration, workforce competencies, clinical governance arrangements and capacity to deliver safely. Where a participant requires supports outside our scope, we work with the participant and their representatives to identify an appropriate provider.
Our support workers can carry out basic bowel care, urinary catheter care and PEG feeding. We do not provide supra-pubic catheter care, catheter insertion, complex bowel care involving non-routine treatment, feeding services for a frequently dislodged feeding tube, or support for a person at high risk of choking.
