Our specialist focus. Support Foundation provides disability-related NDIS supports for eligible participants who are involved with, or transitioning from, the justice system. We work collaboratively with participants, families, support coordinators, justice agencies, health services and other stakeholders to support safe transitions, daily living, community participation, accommodation pathways and greater independence.

We see the person first. Our role is to deliver the disability supports in a participant’s NDIS plan in a consistent, respectful and well-coordinated way, so that the transition back into the community has the best possible chance of lasting.

How we can help

  • Transition planning from custody to community, working with the participant and their team before release
  • Disability support for participants on bail, parole, probation or community-based orders
  • Daily living and Core Supports
  • Community access and participation
  • Accommodation and supported-living transitions
  • Support coordination, where funded in the participant’s plan
  • Psychosocial recovery coaching, where funded in the participant’s plan
  • Working consistently with a participant’s behaviour support plan, in line with NDIS behaviour support requirements
  • Support to attend appointments and coordinate services
  • Skill development and establishing routines
  • Liaison with stakeholders, with the participant’s consent
  • Support to maintain stable community living

Working alongside the justice system

We provide disability-related supports that work alongside justice and mainstream services. The justice system remains responsible for court conditions, supervision, law enforcement and the management of public-safety risks. Our supports focus on the participant’s disability-related needs and NDIS goals, and we keep clear, consent-based communication with the other agencies involved.

Behaviour support and restrictive practices

Where a participant has a behaviour support plan, our team works consistently with that plan. Any regulated restrictive practice is only used where it is authorised and included in the participant’s behaviour support plan, and in line with the requirements of the NDIS Quality and Safeguards Commission.

Who we work with

Participants, families and carers, support coordinators and specialist support coordinators, NDIA planners, correctional and community corrections services, legal and advocacy services, mental health and health services, and housing and accommodation providers.

Referrals and scope

Support Foundation assesses every referral individually. We only accept supports that fall within our NDIS registration, workforce competencies, clinical governance arrangements and capacity to deliver safely. Where a participant requires supports outside our scope, we work with the participant and their representatives to identify an appropriate provider.

Make a referral

Support coordinators, justice and health professionals, families and participants can refer to us. It helps to share the participant’s NDIS plan, expected release or transition date, accommodation situation and any existing support or behaviour support plans.
