Support for NDIS participants who are medically ready to leave hospital and require disability-related assistance to transition safely back home, into temporary accommodation or into an approved longer-term living arrangement.

Who is responsible for what

The health system remains responsible for clinical hospital care and safe discharge planning. NDIS-funded disability supports can then help the participant transition and live in the community. Support coordination may continue during the hospital stay, which helps supports to be ready on the day of discharge.

Support Foundation assesses every referral individually. We only accept supports that fall within our NDIS registration, workforce competencies, clinical governance arrangements and capacity to deliver safely. Where a participant requires supports outside our scope, we work with the participant and their representatives to identify an appropriate provider.
